Fee Code Column Definitions

If a column name appears in yellow highlight, this column is automatically generated programmatically by the Fee Code Service.  Other columns are maintained manually by editing the Excel template spreadsheet.  The manual columns may or may not be used in the generation of the Test Guide output.

A.      Method/Kit: Flag indicating what this line describes.

Values:

K: Method Kit

M: Method

A: Appendage: This says we want to append this row to the last previously encountered M method.  In this case, the Method/Kit Code and the Fee Code are the same as the previous Method/Kit, and all other “yellow” fields are left blank.  This handles the situation where more detailed target descriptions are to be listed in the Test Guide with the same list price and support price.  Note that this is NOT the same as Fee Code Overrides, where a specific sample type and target type is needed to indicate a different list and support price for the same method.

B.      Method/Kit Code: Code value indicating the method kit or method.  This is a key value used in regenerating the Fee Code Template.  The code depends on the flag value in the A column.

C.      Sample Code(s): This only applies in cases where a Fee Code overrides a Fee Code for the associated Method (does not apply to Method Kits as they have their own assignment of Sample Types and Target Types).  Separate codes by semi-colons when there are two or more.  This comes from the Fee Code Override table in the Casebook database [^METHODS(,”FCO”,)].

D.     Target Code(s): Fee Code overrides involve a combination of Sample Code and Target Code. Separate codes by semi-colons when there are two or more.  See “Sample Code(s)” for further information.

E.      Method Kit Name: This is the "Name" of the Method Kit, and remains blank for Methods.

F.       Test Name: This comes from the Fee Code Description.  This value applies to both Method Kits and Methods.  Use <b> </b> formatting rule in the Fee Code description to format the bold part of Test Name.  This field is part of the online Search function.

G.     Method: Value comes from the Name of the Method or Method Kit.

H.     Sample Type: Sample Type and Target values are optional.  They are only required if they have a Fee Code that overrides the Fee Code assigned to the Test/Method (see columns C and D). The values come from the Fee Code Override table ONLY IF one has been defined there.  In most cases it is blank.  Separate values by semi-colons when there are two or more. 

I.        Target: See H.  This is the scientific name coming from the Targets table in the Casebook database.

J.        Fee Code: This is the fee code assigned to the Method Kit or Method, and originates from the Great Plains database.

K.      List Price: This value comes from the Great Plains database.

L.       Support Price: This value comes from the Great Plains database.  There may not always be a Support Price.

M.   Target Desc Breakdown: See column A for more information.  In cases where an existing Method is to be broken down by Target, this semi-colon delimited list itemizes the descriptions needed for extra lines.  Remaining fields remain the same as parent line (i.e. prices).  If this is a Fee Code Override, the description entered here overrides the value for the Test Name.  In this case, follow the description with the Method Code in parenthesis.

N.     Sample/Comment: Comments related to the sample types of the Method or Method Kit.

O.     Keywords: This field is used as part of the online Search function, in addition to the Test name.  Any values may be listed here, separated by semi-colons.

P.      Lab: Optional.  Use Micro, ClinPath or Pathology.

Q.     TAT: Optional.  Description of turnaround time.  Eg. “Same day”, “2-3 weeks”.

R.      Minimum Sample: Optional.  Description of minimum sample.  Eg. “0.2 ml”.

S.       Species: Optional.  List all Species that apply to this Test Code, separated by semi-colons.  If all Species apply, leave blank.

T.      Specimens/Containers: Optional.  Description of specimens/containers.  Eg. “Red, green or purple tube”.

U.     Collection Protocol: Optional.  Description of collection protocol.

V.      Handling/Shipping: Optional.  Description of handling/shipping.  Eg. “Refrigerate.  Ship on ice packs.  Serum or plasma can be frozen.”

W.   Additional Information: Optional.  Any additional information.
